Not another inbox. Not another reporting tool. A prescribing surface that knows your patient's severity, your compliance constraints, and the rehab dose your team is comfortable defending.

BrainMeBack is designed for use under clinician supervision with patients whose cognitive capacity may be impaired. The platform never recommends a treatment directly to the patient — every prescription, severity assessment, and protocol change is the supervising clinician's responsibility.

Reviewed plan assembly

Select multiple protocols or individual modules, then review the assembled plan in one gated screen. Cumulative weekly session load, severity exclusions, and duplicate-exercise merges are all surfaced — nothing is prescribed until you confirm.

Real-time session monitoring

Accuracy, reaction time, engagement, clinician-flagged session events. Tune difficulty mid-program, pause prescriptions in one click, and see who in your caseload is falling behind on compliance.

In-clinic supervised sessions

Beyond at-home prescriptions, you can run a module with the patient in your office on a clinic device — for a baseline session at intake, supervised practice, or guided onboarding for patients without a device at home. A demonstration mode lets you show the patient how the app works without recording anything to their record. Recorded in-clinic sessions feed the same dashboard analytics as at-home use.

Severity-aware prescribing

Every module is gated to the patient’s Rancho Los Amigos level. Difficulty caps prevent prescriptions that would frustrate, agitate, or generate false failure signals.

SMART on FHIR EHR integration (where deployed)

Designed to integrate via SMART on FHIR with major EHRs including Epic, Cerner, VA VistA, and Allscripts. Authentication uses OAuth 2.0 with PKCE; tokens are encrypted at rest; every import is audit-logged with clinician ID and timestamp. Production EHR connections are configured per-deployment and require the hospital’s own EHR app-marketplace or vendor-side approval process. Multi-clinician, multi-tenant

Postgres Row-Level Security enforces per-clinician patient isolation. Sign in as Dr. A, see Dr. A’s caseload — never anyone else’s. Org-scoped policies cover team practice.

Hash-chained audit log

Every PHI access, prescription change, severity reassessment, and protocol application is appended to a hash-chained audit log. Designed to support survey workflows (TJC, CARF, internal audit). Off-platform integrity attestation is on the post-seed roadmap.

A typical day on the dashboard.

    Morning huddle

    Open the dashboard. Clinician-flagged session events and compliance alerts from the last 24h are at the top of the page. Triage which patients need a clinical-review touch today.

    New admission

    Set severity (auto-mapped to Rancho), pick a protocol preset that fits the diagnosis, review the assembled plan, confirm. Patient gets an activation code; their app picks up the prescription on first launch.

    Mid-program adjustment

    A patient plateaus. Open their detail page, see the per-exercise accuracy + d-prime trend, bump difficulty or swap an exercise. The change is audit-logged with your clinician ID.

    Discharge

    Generate a clinical summary, mark prescriptions completed, hand off to the next provider via SMART on FHIR.
